Terms & Conditions* In the interest of maintaining the safety and security of our customers, employees and property Fasturtle Interactive, LLC (the “Company”) may order a “consumer report” (a background report) on you in connection with your employment application, and if you are hired, or if you already work for the Company, may order additional background reports on you for employment purposes. The background report may contain information concerning your character, general reputation, personal characteristics, mode of living, and credit standing. The types of information that may be ordered include but are not limited to: Social Security number verification; criminal, public, educational and, as appropriate, driving records checks; verification of prior employment; reference, licensing and certification checks; credit reports; and drug testing results. The information may be obtained from private and public record sources, including personal interviews with your associates, friends, and neighbors. The nature and scope of the most common form of investigative consumer report is an investigation into your education and/or employment history. I certify that all of the information provided to Fasturtle Interactive, LLC is true and complete to the best of my knowledge. I hereby consent to noted investigations and authorize, without reservation, anyone contacted by Fasturtle Interactive, LLC and/or their agent to furnish information. I agree that a photocopy, fax, or email of this authorization shall be valid as an original. I hereby authorize Fasturtle Interactive, LLC to contact my present employer for employment verification/references. I understand before I am denied employment based, in whole or part, on information obtained in the consumer report, I will begin given a copy of the report and a description in writing of my rights under the FCRA. My acceptance certifies that my responses are true and complete. I understand that misrepresentation or omission of facts is cause for immediate dismissal no matter when the misrepresentation or omission is discovered. Furthermore, my acceptance authorizes investigations of all statements contained in this application. I hereby release prior employers, schools and law enforcement agencies from all liability in responding to inquiries in connection with my application. I understand that, if hired, my employment is "at will" and either I or my employer may terminate my employment at any time, with our without cause, unless the "at will" arrangement is modified by a written agreement signed by both me and a duly authorized officer of the Company. The Company reserves the right to request a random drug test without cause or if you employee shows signs of impairment on the job. Employment may be terminated upon positive results. If positive drug test is obtained employee may not be eligible for Worker's Compensation and/or unemployment benefits. The Company reserves the right to run an MVR if employee is authorized to drive company vehicle. All employees will enter their employment with a ninety (90) day probation period, in which the employee may be evaluated for the continuation of satisfactory employment.
